Yes, piles and fissures can be treated in the same operative sitting. • Grade determines treatment: Grade 1–2 haemorrhoids with an acute fissure are managed differently from grade 3–4 haemorrhoids with a chronic fissure. Each condition’s severity independently determines the procedure.
• Imaging: Routine imaging isn’t required. It’s added when a fistula is suspected, inflammatory bowel disease is in the history, or examination findings don’t match the clinical picture.

• Procedure combination: Laser haemorrhoidoplasty or stapled haemorrhoidopexy for piles is performed alongside lateral internal sphincterotomy for the fissure in the same anaesthetic. The sphincterotomy relieves elevated internal sphincter tone that contributes to both fissure non-healing and haemorrhoidal engorgement.
• Botulinum toxin alternative: Where lateral internal sphincterotomy carries continence risk, botulinum toxin injection is used instead. It achieves temporary sphincter relaxation sufficient for fissure healing without permanent sphincter division.
• Recovery timeline: Mild to moderate perianal discomfort for five to seven days, stool softeners throughout, sitz baths twice daily, and return to desk work within one to two weeks for most patients.
